Key Responsibilities:- Provide specialized assessment, coordination, and treatment for individuals living with mental health issues.
- Conduct bio-psychosocial assessments and design person-centered, strength-based change strategies.
- Work collaboratively with an inter-professional team to establish and maintain partnerships with related community organizations.
- Provide integrated model of care, linking clients to primary care and fostering existing relationships with primary care.
- Master's Degree in Nursing or recognized equivalent.
- Current registration in the Extended Class with the College of Nurses of Ontario.
- Nurse Practitioner's Certificate from a recognized program.
- Five years of experience working in the field of Mental Health, with a focus on Children & Adolescent and Eating Disorders.
- Expertise in conducting bio-psychosocial assessments and strong formulation skills.
- Experience in motivational interviewing and trauma-informed care.
- A dynamic and supportive work environment.
