Job Title: Registered Practical Nurse

Job Summary:

We are seeking a highly skilled Registered Practical Nurse to join our team at St. Joseph's Care Group. As a Registered Practical Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities:

  • Provide direct patient care, including physical assessments, medication administration, and wound care.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct community assessments, psychosocial rehabilitation, and crisis management.
  • Work effectively with people with serious mental illness and intellectual disabilities, as well as concurrent addiction issues.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.

Requirements:

  • Current registration with the College of Nurses of Ontario.
  • Scope of Practice must include physical assessment and pharmacology.
  • Basic Cardiac Life Support and First Aid Certificates.
  • Experience working with people with serious mental illness and intellectual disabilities, as well as concurrent addiction issues.
  • Excellent communication, problem-solving, and interpersonal skills.
  • Class G Driver's License and access to a reliable vehicle with adequate insurance.

Condition of Employment:

  • This position is conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated.
  • Offers of employment made to external candidates are conditional upon providing proof of COVID-19 vaccination or valid medical exemption.
