Job Title: Community Registered Nurse

We are seeking a highly skilled and compassionate Community Registered Nurse to join our team at Closing the Gap Healthcare. As a Community Registered Nurse, you will be responsible for providing high-quality care to our clients in their homes and communities.

Key Responsibilities:
  • Provide comprehensive care to clients in their homes and communities
  • Assess and manage client needs, including wound care management
  • Administer IV antibiotics, cannulation, and other medical treatments as required
  • Coordinate with healthcare teams and report to the HCCSS (LHIN)
  • Travel between clients and work independently with minimal supervision
Requirements:
  • Current certification as a Registered Nurse (RN) with the CNO
  • Minimum of two years' experience in community, home-based, or visiting nurse related roles
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
