Role Summary:
The Nurse Practitioner plays a vital role in our community, significantly influencing the lives of residents and fostering meaningful human connections.
Key Responsibilities:
- Delivering primary healthcare services in alignment with established policies, the resident care plan, and within the designated scope of practice.
- Collaborating with physicians to monitor diagnosed conditions and provide on-call support as necessary.
- Offering nursing consultations to residents, their families, healthcare professionals, and organizations.
- Engaging in the identification, analysis, and interpretation of trends in resident care outcomes and nursing practice issues to prioritize educational initiatives.
- Addressing concerns raised by residents and family members promptly, mediating satisfactory resolutions, and implementing action plans to prevent recurrence.
- Executing additional responsibilities as assigned.
Essential Qualifications:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class or RN (EC).
- Proven experience in gerontology.
- Advanced skills in assessment, diagnosis, and healthcare management.
- In-depth knowledge of primary healthcare issues in Long Term Care settings.
- Exceptional interpersonal and leadership abilities.
- All candidates must successfully complete the required Vulnerable Sector Check.
Preferred Qualifications:
- Certification from the Canadian Nurses Association is preferred.
