Job Summary
The Mon Sheong Foundation is seeking a highly skilled and experienced Nutrition Care Specialist to join our team. As a key member of our healthcare team, the successful candidate will be responsible for providing high-quality nutrition care services to our residents.
Main Responsibilities
- Develop and implement nutrition care plans that meet the individual needs of our residents, ensuring compliance with relevant legislation and regulations.
- Oversee the menu planning process, ensuring that all meals meet the nutritional needs of our residents and are prepared in accordance with established standards.
- Collaborate with the healthcare team to identify and address nutrient-related health problems, and develop treatment plans to address these issues.
- Conduct regular assessments of residents' nutrition care needs, and make recommendations for changes to their care plans as needed.
- Participate in multidisciplinary care conferences to discuss the nutritional aspects of individual residents and revise their care plans accordingly.
- Develop and maintain relationships with healthcare providers, community agencies, and other stakeholders to ensure that our residents receive comprehensive and coordinated care.
Requirements
- Eligibility to register with the College of Dietitians of Ontario.
- Three years of previous professional experience in long-term care, with a focus on nutrition care.
- Excellent leadership, communication, and interpersonal skills, with a strong interest in gerontology.
- Ability to work independently and as part of a team, with a high level of professionalism, energy, and resourcefulness.
- Sensitivity to the cultural, spiritual, and physical needs of our residents.
