About the Role

We are seeking a skilled and compassionate Personal Support Worker to join our team at Transitions Care, Inc. as a Nursing Care Specialist. In this role, you will provide high-quality care and support to patients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Accompany patients on outdoor activities and provide assistance as needed.
  • Assist with cleaning, sterilizing, and setting up medical equipment to maintain a safe and hygienic environment.
  • Perform various tasks related to patient care and comfort, including bathing, dressing, and grooming.
  • Supply and empty bed pans and maintain patients' rooms to a high standard.
  • Take patients' vital signs and report any concerns to the healthcare team.
  • Prepare and serve meal trays and assist patients with feeding.
  • Transport patients between care facilities and provide support during exercise routines.
  • Collect specimens and administer first aid in emergency situations.
Requirements
  • Secondary (high) school graduation certificate and a willingness to learn and train.
  • First Aid Certificate and CPR Certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills to build strong relationships with patients and colleagues.
Work Environment

This is a temporary, on-the-road position with a variable schedule. You will be working with a variety of patients in different locations, requiring flexibility and adaptability.

Language

The primary language of this role is English.
