We are seeking a highly skilled and dedicated Pharmacist to join our team at Lakeridge Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Provide direct patient care, working closely with patients and the healthcare team to resolve and prevent drug-related problems.
- Participate in patient education, teaching patients and other healthcare professionals about medication use and management.
- Facilitate medication reconciliation activities, ensuring accurate and safe medication use.
- Support the drug distribution system, ensuring timely and accurate medication delivery.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Current registration in good standing with the Ontario College of Pharmacists in Part A of the register.
- Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values.
- Strong clinical assessment skills in identifying and solving drug-related problems.
- Excellent medication counseling skills.
- Demonstrated ability to research and provide drug information effectively to patients and other members of the multidisciplinary team.
- Demonstrated excellent organizational skills.
- Demonstrated ability to work effectively within an interprofessional team.
- Demonstrated effective interpersonal and verbal and written communication skills.
- Proven ability to exercise independent judgment and to assess situations and problems efficiently and effectively.
- Previous hospital experience desired.
- Demonstrated commitment to our core values of Compassion, Innovation, Courage, and Trust.
- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff, patient, and environmental safety.
The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.
