CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse. As a Visiting Nurse, you will have the opportunity to provide exceptional nursing services to patients in their homes, clinics, and other community settings.
Key Responsibilities- Develop and maintain effective care plans for patients in community health settings
- Provide nursing services to patients in a variety of settings, including in-home, clinic, and retirement home settings
- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team
- Perform other clinical duties as required

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- Vulnerable sector check and clear background check
