Job Summary
Vancouver Coastal Health is seeking a Registered Psychiatric Nurse or Registered Nurse to join the Central Intake team at the Pacific Spirit Community Health Centre in Vancouver.
Key Responsibilities
- Conduct centralized intake functions for designated site/program services to ensure linkages between client and appropriate resources.
- Perform initial screening, triage, and referrals based on client needs, urgency of request, and best practices to determine appropriate treatment.
- Provide information to health providers and the general public, establish and enhance relationships/communication within designated program services and other community resources.
- Prepare and maintain client records, maintain and provide accurate intake/referral data and statistics, participate in team meetings and continuing education, and engage in ongoing team planning and quality improvement.
Qualifications
Education & Experience
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years recent related experience in the designated clinical program area or an equivalent combination of education, training, and experience.
Knowledge & Abilities
- Broad knowledge of nursing theory and practice within a client and family-centered care model.
- Broad knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.
- Demonstrated ability to provide client-focused, trauma-informed, harm reduction, and culturally safe care in completing initial client assessments and interventions.
- Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
- Knowledge of community resources available for client/family support and related health services.
- Broad knowledge of other health care disciplines and their role in client care.
- Broad knowledge of adult education principles, methods, and tools.
- Demonstrated ability to problem-solve using sound judgment in applying critical thinking skills within safe limits of client care.
- Demonstrated ability to communicate effectively, both orally and in writing.
- Demonstrated listening and information-seeking skills that promote communication and lead to a co-operative approach to problem-solving within a multidisciplinary setting.
- Demonstrated ability to establish workload priorities.
- Demonstrated ability to deal effectively with conflict situations.
- Demonstrated ability to work both independently and collaboratively as a member of a multidisciplinary team.
- Demonstrated ability to adjust to new or unexpected events.
- Ability to operate related equipment.
- Physical ability to perform the duties of the position.
- Demonstrated computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet, and e-mail software.
