Key Responsibilities:- Assess patients' physical status, functional abilities, needs, and goals through thorough evaluations and consultations.
- Develop and implement evidence-based treatment plans to achieve optimal health outcomes, working collaboratively with the interprofessional care team.
- Provide education and guidance to patients and caregivers on self-management, health promotion, and injury/disability prevention.
- Participate in the development and implementation of care plans, ensuring seamless quality patient care and effective communication with the care team.
- Evaluate and document the effectiveness of interventions, making adjustments as needed to achieve optimal functional independence and physical performance.
- Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient autonomy and dignity.
- Maintain accurate and detailed records of patient progress, treatment plans, and test/treatment results.
- Current full registration with the College of Health and Care Professionals of BC.
- Knowledge of physiotherapy theory and practice within a patient and family-centered model of care.
- Ability to communicate effectively and respectfully with patients, caregivers, and healthcare professionals.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Basic knowledge of research process and methodology.
