- Develop and implement individualized care plans to meet the unique needs of clients/patients/residents.
- Conduct thorough assessments and evaluations to identify client/patient/resident needs and develop effective care strategies.
- Communicate effectively with clients/patients/residents, families, and healthcare team members to ensure seamless care delivery.
Requirements:
- Practical Nursing diploma from a recognized institution.
- Licensed Practical Nurse (LPN) registration with the College of Licensed Practical Nurses of SK (CLPNS).
- Valid driver's license, where required.
- Excellent leadership, interpersonal, communication, and organizational skills.
- Ability to work independently and as part of a team.
