Position Overview

Vancouver Coastal Health (VCH) is seeking a dedicated Registered Nurse to take on the role of Clinical Coordinator within the Health Promotion team.

As a Clinical Coordinator specializing in Home Support, your responsibilities will include:

  • Leading, supervising, and guiding designated personnel while coordinating work activities to ensure high-quality care and effective use of client care resources.
  • Addressing and resolving customer service and client relations challenges effectively.
  • Maintaining a safe and healthy environment for both clients and staff.
  • Providing clinical leadership and oversight to staff to ensure excellence in client care.
  • Assisting management with various administrative tasks and delivering direct health care services to clients as necessary.

About Home Health Programs:

Home Health programs deliver a variety of healthcare and support services to individuals in the community with acute, chronic, palliative, or rehabilitative health care needs.

Client Demographics:

Clients in Home Health typically face ongoing health challenges and/or complex personal and social issues, necessitating careful care planning and resource coordination. This includes frail elderly individuals, younger clients with disabilities, and adults with chronic illnesses who require support to maintain or enhance their independence.

Qualifications

Education & Experience

  • Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of three (3) years of recent clinical experience relevant to the specific clinical area and two (2) years of recent supervisory experience, or an equivalent combination of training, education, and experience.
  • A valid BC Driver's License is required, as local travel may necessitate the use of a personal vehicle.

Knowledge & Skills

  • In-depth understanding of community home health practices within a population health framework.
  • Familiarity with the BCCNM Standards for Nursing Practice.
  • Comprehensive knowledge of clinical practices related to care management and interventions for designated client populations.
  • Awareness of community resources available for client and family support.
  • Proven ability to lead, supervise, and direct staff effectively.
  • Strong communication skills, both verbal and written, with the ability to manage effective interactions with various stakeholders.
  • Demonstrated ability to adapt to new situations and manage psychosocial and behavioral issues.
