Job Summary

We are seeking a highly skilled and compassionate Outreach Worker to join our team at Fraser Health. As an Outreach Worker, you will play a critical role in providing support and services to individuals struggling with substance use issues.

Key Responsibilities
  • Provide culturally sensitive support and services to clients, including harm reduction strategies and overdose prevention education.
  • Work collaboratively with clients to identify and address their unique needs and goals.
  • Develop and maintain relationships with community resources and agencies to ensure seamless referrals and support.
  • Administer medication to clients under the direction of a Registered Nurse.
  • Provide basic health-promoting information and promote development of life-skills to clients.
  • Perform a variety of general clerical duties, including responding to calls and inquiries, maintaining client files, and utilizing computer software applications.
Requirements
  • Completion of Substance Use Certificate, plus one year's recent related experience working with clients with substance use issues.
  • Knowledge of Indigenous health approaches, protocol, and cultural issues.
  • Valid BC driver's license and vehicle.
  • Current CPR certificate.
Working Environment

This role will primarily work in a community setting, providing outreach services to clients in their homes or in community locations. The successful candidate will be required to work independently and collaboratively as part of a multidisciplinary team.
