Home and Community Care Support Services is seeking a skilled Mental Health and Addictions Nurse to collaborate with district school boards across Ontario. In this vital role, you will play a key part in supporting students' mental health and addressing addiction issues within educational settings.
Your Responsibilities- Provide expert guidance to educators on recognizing and responding to student mental health and addiction concerns.
- Develop individualized care plans in collaboration with a multidisciplinary team of professionals, ensuring comprehensive support for students facing mild to complex mental health and substance abuse challenges.
- Assist school boards in implementing effective strategies to promote positive mental health and address addiction needs within the school community.
- Offer direct clinical services to students, including medication management, crisis intervention, and support for self-harm or treatment refusal.
- Facilitate the smooth transition of students back to school following hospitalization by developing tailored plans and coordinating care with relevant stakeholders.
- Active membership in good standing with the College of Nurses of Ontario.
- Minimum 3-5 years of experience as a Registered Nurse, with a demonstrated focus on mental health and/or addictions care for children and youth.
- Comprehensive understanding of community resources and the roles of various healthcare professionals.
- In-depth knowledge of the healthcare delivery system and available community support services.
- Advanced assessment and diagnostic reasoning skills, coupled with exceptional interpersonal, communication, problem-solving, and decision-making abilities.
- Proven ability to effectively manage time, prioritize tasks, and work both independently and collaboratively in a fast-paced environment.
- Proficiency in accurately completing documentation, reports, and forms.
- Valid driver's license and access to a reliable vehicle.
