Community Inclusion Specialist

Pacific Coast Community Resources is seeking a skilled Community Inclusion Worker to join our team. As a Community Inclusion Worker, you will play a vital role in supporting individuals with disabilities to participate in community-based activities and achieve their goals.
Key Responsibilities- Advocate for the rights and choices of the individuals we support
- Build positive relationships with individuals, families, and community members
- Coordinate and facilitate community-based activities and events
- Develop and implement individualized service plans
- Record and maintain accurate documentation of individual progress
- Participate in ongoing training and professional development
- Grade 12 education
- Community Support Worker Diploma or equivalent
- Minimum 2 years of experience in community social services/health care
- Basic American Sign Language proficiency
- Current Emergency First Aid/CPR and Food Safe certificates
- Behavior management training
- Excellent communication and interpersonal skills
- Competitive salary: $25.95 – $29.76 per hour
