As a Community Foot Care Specialist, you will be engaged in providing essential foot care services both in our clinic and through home visits, ensuring patients receive the highest quality of care.

Key Responsibilities

  1. Conduct scheduled community visits to address patient needs within your professional scope
  2. Deliver ethical and safe nursing care
  3. Utilize appropriate assessment tools to evaluate individual patient requirements
  4. Implement and continuously review care plans, making adjustments as necessary
  5. Engage with patients through a collaborative approach, fostering trust and rapport
  6. Maintain and update professional records related to patient interactions and care plans

Qualifications

  1. Current registration with the CLPNA as a Licensed Practical Nurse in good standing
  2. Possession of an Advanced Foot Care Certificate
  3. Valid driver's license and access to a vehicle are mandatory
  4. Experience in community care is preferred
  5. Dependability and adaptability
  6. Willingness to travel to patient residences within a designated area
  7. Experience working with vulnerable populations
  8. Commitment to making a positive impact in the community
  9. Availability for on-call duties, including after-hours, to ensure patient safety and care needs are met
