Job Summary
We are seeking passionate and caring Acute Care Community Nurses to join our team. As a member of our team, you will use your clinical skills to ensure that our clients receive the highest quality care, delivered with compassion, excellence, and reliability.
Responsibilities
- Provide high-quality care to clients in their homes, using your clinical skills and expertise.
- Work collaboratively with our interdisciplinary team to develop and implement care plans that meet the unique needs of each client.
- Conduct thorough assessments of clients' physical and emotional needs, and develop care plans that address these needs.
- Provide education and support to clients and their families, to promote independence and self-care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal client outcomes.
Requirements
To be successful in this role, you will need:
A current registration with the provincial licensing body.A valid driver's license and reliable vehicle.Excellent communication and interpersonal skills.A strong commitment to delivering high-quality care and excellent customer service.What We Offer
