Key Responsibilities:

  • Evaluate health outcomes and develop effective care plans

  • Provide cost-effective, organized, and outcome-directed care
  • Document client information and care plans accurately and thoroughly

Requirements:
  • Current registration as a Registered Nurse in the province of practice
  • Canadian Nurses Association certification in Community Health Nursing (an asset)
  • Experience in community health (an asset)
  • Strong written and verbal communication skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and handle sensitive information
