Job Summary

The Home Support Worker provides personalized care and support services to clients and families within the framework of VON policies, procedures, and standards. This role involves collaborating with healthcare professionals to meet the needs of clients and promote their independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Develop and implement personalized care plans in collaboration with the Personal Support Program Manager.
  • Provide personal care services, including light housekeeping, meal preparation, and assistance with daily living activities.
  • Monitor and maintain household safety, and report any concerns to the supervisor or Personal Support Program Manager.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in multidisciplinary teams to provide comprehensive care and support services.
  • Perform administrative tasks, such as completing daily call checks and other forms as required.
Requirements
  • High school diploma or equivalent.
  • CCA Certification (Nova Scotia) or equivalent.
  • Personal Support Worker program or Health Care Aide certification.
  • Standard First Aid Certificate or CPR level C Certificate (if applicable).
  • Experience working with elderly or ill individuals or children.
  • Strong communication and assessment skills.
  • Awareness of community resources.
  • Ability to work effectively in a team environment.
Work Environment

This role requires working in a fast-paced environment, with attention to detail and the ability to lift and carry equipment as needed. The Home Support Worker must be able to wear Personal Protective Equipment (PPE) and have access to a personal vehicle for work.
