Job Summary:

We are looking for a dedicated and compassionate Licensed Practical Nurse to provide nursing assessments, case coordination, and direct care to clients in their homes or in the community. The successful candidate will have a strong background in home care or acute care, with experience in wound care, skin care, and ostomy care.

Responsibilities:
  • Provide nursing assessments and develop care plans to meet the unique needs of each client
  • Coordinate and provide direct care to clients in their homes or in the community
  • Work collaboratively with clients, families, and healthcare teams to ensure seamless care
  • Use critical thinking skills to assess, plan, and evaluate client care
  • Stay up-to-date with the latest evidence-based practices and guidelines
Requirements:
  • Completion of an accredited practical nurse education program
  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Two years of recent home care or acute care experience in the last five years
  • Advanced wound care knowledge and experience required
Preferred Qualifications:
  • Experience working with acute clients and/or long-term clients with a focus on chronic disease management
  • Experience in providing case management services
  • Experience in Home Parenteral Therapy (HPT), wound, skin, and ostomy care, palliative support, and client teaching
