Key Responsibilities:
  • Assess and Evaluate: Conduct comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develop and Implement Care Plans: Develop individualized plans of care that incorporate analysis of data and current scientific findings.
  • Provide Interventions: Provide interventions with a focus on achieving realistic client outcomes.
  • Monitor and Report: Report changes to update the plan of care to reflect progress towards goals and outcomes.
  • Medication Administration: Administer medications as needed.
  • Record Keeping: Maintain client records showing systematic assessments, planning interventions, and evaluations.
  • Referrals: Initiate referrals as needed to other health team members.
Requirements:
  • Education: Graduate of an accredited school of professional nursing.
  • Licensure: Current license as a Registered Nurse.
  • Certification: Current CPR certification.
  • Transportation: Access to reliable transportation.
  • Confidentiality: Maintains confidentiality in all aspects of the job.
  • Safety and Infection Control: Knowledge of safety and infection control practices.
