The Long-term Care Coordinator (LtCC) plays a pivotal role in ensuring the seamless delivery of individualized care within designated clinical areas. This position supports the efficient movement of individuals through the healthcare system, facilitating access to necessary resources. As a supervisor and role model, the LtCC provides clinical guidance, conducts assessments, and educates families on care options.

Key Responsibilities
  • Coordinate day-to-day operations and quality individual care within designated clinical areas
  • Supervise and serve as a resource to staff for clinical decision-making and assessments
  • Review, analyze, and make recommendations on administrative systems, including policies, procedures, and staffing levels
