Nurse Case Manager

Job Summary:
Alberta Health Services is seeking a skilled Registered Nurse to join our Community Care Registered Nurse Case Management Team. As a member of this team, you will be responsible for assessing, designing, and implementing continuing care services for a caseload of clients residing in the community.
Key Responsibilities:
- Conduct comprehensive assessments of clients to identify their healthcare needs and develop individualized care plans.
- Provide consultation, assessment, care, and health teaching to clients in various living settings as required.
- Collaborate with other healthcare professionals to ensure seamless care coordination and delivery.
- Develop and maintain knowledge of current trends, issues, and practices in community care.
- Stay up-to-date with best practices in case management processes and quality improvement methodologies.
- Apply conflict resolution and crisis intervention skills to resolve client issues.
- Lead and manage a caseload of clients, prioritizing tasks and managing time effectively.
Requirements:
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Recent geriatric clinical experience (preferred).
