The University Health Network is seeking an experienced Nurse Practitioner to join our team in the Geriatric Psychiatry Services at the Toronto Rehabilitation Institute. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care, developing and implementing care plans, and collaborating with healthcare professionals to achieve optimal patient outcomes.
Key Responsibilities
- Assess and diagnose patients with complex geriatric conditions, including dementia and behavioral symptoms
- Develop and implement individualized care plans, incorporating pharmacological and non-pharmacological interventions
- Collaborate with geriatric psychiatrists, psychologists, and other healthcare professionals to provide comprehensive care
- Conduct research and quality improvement activities to enhance patient care and outcomes
- Participate in education and professional development activities to stay current with best practices and evidence-based care
Requirements
- Master's degree in Nursing
- Current registration with the College of Nurses of Ontario in the Extended Class
- Nurse Practitioner (NP) Adult certificate or Primary Care certificate
- Minimum 3 years of experience working in geriatric care, with a focus on mental health
- Expertise in managing behavioral and psychological symptoms of dementia
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced, dynamic environment
