Job Title: Visiting Nurse
Job Summary:
Closing the Gap Healthcare Group is seeking a skilled Visiting Nurse to join our team. As a Visiting Nurse, you will provide high-quality care to clients in their homes, focusing on wound care management and other community-based services.
Key Responsibilities:
- Provide comprehensive wound care management to clients in their homes
- Administer IV antibiotics, cannulation, and other treatments as required
- Coordinate with healthcare teams and report to the HCCSS (LHIN)
- Travel between clients to provide care and support
- Document client progress and update reports
- Order supplies and equipment for patients
- Build strong relationships with clients and their families
- Work independently and as part of a team to achieve client goals
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) certification
- Minimum 2 years' experience in community or clinical/hospital setting
- Current Standard First Aid & CPR/AED (Level C) certification
- Mask Fit test certification
- Ability to work weekends and on-call
