POSITION OVERVIEW:
The Hospice Care Manager is tasked with the recruitment and leadership of the nursing care team dedicated to delivering exceptional care and services to Residents. This role involves effective communication with Residents' families, healthcare providers, and representatives from health authorities. The Manager coordinates and oversees the nursing team, providing necessary guidance, direction, and discipline to ensure the delivery of individualized, Resident-focused care based on assessed needs and care plans. The Hospice Care Manager exemplifies knowledge of leadership and gerontological best practices in alignment with provincial regulations and Park Place policies.
KEY RESPONSIBILITIES:
- Recruit, orient, lead, and supervise the nursing care team to ensure comprehensive and consistent Resident care planning and delivery, adhering to Residents' preferences and Park Place policies.
- Provide leadership through a collaborative approach, monitoring and directing the care team in their provision of care. Facilitate orientation for new nursing staff and conduct regular performance evaluations. Offer ongoing education and mentorship to nursing personnel, addressing performance issues as necessary.
- Serve as a role model by demonstrating professionalism, empathy, and respect in all interactions, while recognizing the impact of personal behavior on team dynamics.
- Establish and maintain therapeutic relationships with Residents, families, and team members through effective communication and interpersonal skills.
- Ensure appropriate staffing levels and monitor team collaboration to maintain quality Resident-centered care, addressing any issues that arise.
- Collaborate with leadership to spearhead quality improvement initiatives and performance measurement processes, ensuring compliance with best practices and accreditation standards.
- Focus on enhancing care quality and safety by taking an active role in site committees related to nursing practices and therapeutic protocols.
- Ensure all documentation related to assessments, observations, care delivery, and Resident responses meets professional standards and adheres to Park Place policies.
- Lead the nursing team in planning, organizing, and prioritizing care tasks, utilizing resources effectively, and adapting to changing Resident needs.
- Promote safety within the nursing team by emphasizing accident prevention, knowledge of emergency procedures, and adherence to infection control protocols.
- Participate in educational programs to maintain clinical competence and knowledge.
- Perform additional related duties as assigned.
HEALTH AND SAFETY COMMITMENTS:
- Understand and comply with Park Place and provincial Occupational Health and Safety policies.
- Educate staff on workplace safety issues, including violence and harassment prevention.
- Demonstrate leadership in promoting and monitoring safe work practices.
- Take all reasonable precautions to ensure the safety of Residents, staff, and the public.
- Report any incidents or unsafe conditions promptly.
- Lead by example in maintaining a safe work environment.
QUALIFICATIONS:
Must hold a valid Registered Nurse designation.
SKILLS AND COMPETENCIES:
- Fluent communication in English, both written and verbal.
- Demonstrated leadership skills with the ability to motivate and guide staff.
- Strong problem-solving abilities with critical thinking and decision-making skills.
- Experience in clinical problem-solving within gerontological care.

- Proficient in using relevant technology and nursing care equipment.
