About the Role
We are seeking a skilled Social Worker to join our Community Wellness team at Pan Am Clinic. As a Social Worker - Community Wellness Specialist, you will play a vital role in providing client care and promoting wellness, disease prevention, and chronic disease management.
Key Responsibilities
- Provide a diverse range of client care services, including extended community outreach, health education, case management, treatment support, counseling, group facilitation, community programming, and advocacy.
- Work collaboratively with the interdisciplinary Community Wellness team to ensure client engagement and reduce or remove barriers.
- Develop and implement harm-reduction approaches to client care, consistent with the philosophy of harm-reduction.
- Provide education and support to clients on health and wellness topics, including disease prevention and chronic disease management.
- Facilitate sharing circles and group discussions to promote health and wellness.
Requirements
- Master's degree in Social Work or a related field.
- Minimum of 5 years of experience working as a Social Worker, with a minimum of 3 years in the areas of addictions, mental health, crisis intervention, and cognitive behavioral therapy.
- Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills.
- Excellent verbal and written communication skills and proficiency in the use of computer systems.
- Knowledge and understanding of Indigenous culture and traditions, and the impact of colonization on the health and well-being of Indigenous individuals and families.
Preferred Qualifications
- Experience working in an inner-city community setting.
- Knowledge and experience working with students and/or volunteers.
- Links/relationships with community resources.
- Experience working with service-specific computer programs such as EMR, eChart, and Accuro.
