About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Fraser Health Authority. As a key member of our multidisciplinary team, you will provide high-quality patient care and work collaboratively with healthcare professionals to deliver exceptional patient outcomes.

Responsibilities
  1. Conduct comprehensive patient assessments to identify strengths and limitations, and develop individualized care plans to address their needs.
  2. Provide supportive care to patients, promoting their involvement in health care decisions and respecting their personal choices.
  3. Collaborate with the healthcare team to develop and implement care plans, and participate in multidisciplinary care planning meetings and family conferences.
  4. Administer medications and perform other nursing interventions as required, and document patient care and outcomes.
  5. Assist with admissions, discharges, and transfers, and maintain a safe and clean environment for patients and staff.
  6. Participate in continuous quality improvement activities, education sessions, and team conferences to enhance patient care and outcomes.
Requirements
  • Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Ability to communicate effectively, both verbally and in writing, and to work collaboratively with the healthcare team.
  • Knowledge and competencies to work at full scope of practice, including administering medications.
