We are seeking an experienced Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility
  • Provide access and referrals to community services
  • Engage with patients, caregivers, and other healthcare practitioners
  • Develop and implement treatment plans to meet patient needs
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Maintain accurate and up-to-date patient records
  • Participate in professional development activities to stay current with best practices

Requirements:

  • Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
  • Related professional experience in a Physiotherapy role
  • Knowledge of healthcare legislation and policies
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
