Job Title: Client Care Coordinator

We are seeking a highly skilled and detail-oriented Client Care Coordinator to join our team at Spectrum Health Care. As a Client Care Coordinator, you will play a vital role in providing exceptional service and support to our internal and external clients.

Key Responsibilities:
  • Collaborate with community PSWs and Nurses to schedule and coordinate services
  • Liaise with clients, staff, and community partners to ensure seamless communication
  • Act as a problem solver for clients and respond to inquiries from HCCSS and employees
  • Report client and staff concerns to the Home Support Supervisor
  • Liaise with HCCSS representatives regarding client care
  • Collect and enter client data into the computer system
  • Provide administrative support to the personal support worker and Nurse program
Requirements:
  • Minimum of 2 years of experience in an office setting
  • Ability to work weekends
  • Excellent English written and verbal communication skills
  • Ability to multitask and prioritize
  • High school diploma or equivalent
  • Proficient in Word, Outlook, and ability to learn proprietary software
