Key Responsibilities- Provide comprehensive primary health care to patients, including health assessments, diagnosis, and treatment planning.
- Work collaboratively with our physicians and other healthcare professionals to ensure seamless patient care.
- Develop and implement individualized health plans, taking into account patient needs, preferences, and goals.
- Conduct health education and promotion activities to empower patients to take an active role in their health.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current registration as a Registered Nurse in the extended class with the Colleges of Nurses of Ontario.
- Masters Degree in Nursing.
- Current CPR and BCLS certification.
- Experience working in a family practice, community, or primary health care setting.
- Excellent health assessment and clinical skills, with the ability to work independently and as part of a team.
- Competitive hourly rate of $68.00.
- 37.5 hours per week, with opportunities for additional hours.
