Key Responsibilities
• Provide patient-focused care to enhance patient outcomes
• Conduct thorough pharmacotherapy assessments and medication reconciliations
• Facilitate drug distribution and procurement processes
• Train and mentor new pharmacy staff and learners
• Communicate effectively with healthcare teams
Requirements:
• Bachelor's or Doctor of Pharmacy degree required
• Active Ontario pharmacy registration and competency certificate
• Hospital pharmacy experience preferred
• Excellent communication and organizational skills
• Clinical experience in patient-centered rehab care an asset Empower patients through effective medication management and collaboration at UHN.
