Yorktown Family Services is a community-based non-profit social services agency dedicated to providing effective, accessible, quality mental health treatment, prevention, and outreach services to children, youth, women, and families in West Toronto. We are uniquely organized to deliver services through two independent but integrated organizations: Infant, Child, and Youth Mental Health & West Toronto Youth Hub — a children’s mental health center, and Violence Against Women Services — a shelter for women and their children who are fleeing an abusive relationship.

Primary Function:

Yorktown Family Services is seeking a therapist with experience in Care Navigation to join our dedicated programs at Yorktown. The Care Navigator Triage role will provide care navigation support, carry a brief service caseload, and complete assessment screeners for our Rapid Access program located at The West Toronto Youth Wellness Hub. The candidate will assist with triage for children, youth, young adults, and their families who require complex care, mental health, and social service support within the integrated care team at Yorktown.

Responsibilities:

- Will provide client advocacy, taking and receiving referrals, prioritizing needs, and ensuring a smooth delivery of appropriate service.
- Provide triage support for our walk-in/rapid access clients by supporting them in completing registration forms and providing clients with an initial assessment with a focus on social service support and preparing them for a therapy session.
- Adapt and cover shifts at all locations when required.
- Facilitate groups when required.
- Keep electronic health records up to date as per agency and professional college requirements.
- Act as a single point of contact for integrated care services.
- Work closely with healthcare partners to support the success of patients exiting hospital psychiatric settings and living in the community.
- Support general intake functions when the intake worker is on vacation.
- Provide brief SFBT counselling within our walk-in and virtual Rapid Access Therapy and at the West Toronto Youth Hub clinic, and in the community as needed.
- Engage in service coordination and case consultation meetings with partnering agencies and external services to ensure successful service transitions.
- Manage a small caseload of clients who require Care Navigation and are connected to our Rapid Access program.
- Administer program and process evaluations and data collection for internal and external reporting.
- Present clients at case review meetings.
- Provide Care Navigation services for unattached clients who use our Urban Tele-Mental Health Program.
- Ensure agency health and safety protocols are followed.

Qualifications:

- M.S.W., MACP or equivalent Master’s degree
- Registration or eligibility for registration with a relevant college is required
- Minimum two years ofrelevant children and youth mental health experience, with knowledge and expertise using evidence-based clinical models such as CBT and TF-CBT, goal-focused and brief therapy models of service provision.
- Strong knowledge of and experience working with children, youth, and young adults who are experiencing challenges relating to mental health, addictions, trauma, and other mental health issues, and/or have multiple risk factors, limited protective factors
- Knowledge of and demonstrated skill in youth engagement principles and resiliency models
- Strong knowledge and experience working in the West End of Toronto, including within NIAs (Neighbourhood Improvement Areas), as well as other identified communities and/or populations
- Demonstrated ability to work cooperatively and negotiate effectively within a multi-disciplinary team of service providers, as well as the ability to work independently
- Excellent oral and written communication skills
- Superb interpersonal and organizational skills
- Ability to work flexible hours, including evenings, is essential
- Access to a reliable vehicle is a definite asset
- Fluency in a second language is an asset
- Complete a VSC
