Detailed Overview
Reporting to the Program Manager, with accountability for clinical Social Work Practice to the Clinical Practice Leader, Social Worker, provides therapeutic clinical counseling services to patients and families in the assigned clinical area within Acute Care including the surgical, general medicine, palliative, and geriatric programs. Conducts social work assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices and provides consultation to patient, families and healthcare professionals. Provides leadership within the interdisciplinary team in care planning for the effective utilization of patient resources and participates as a member of local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery to enhance patient care services and reduce gaps in the community. Participates in in-services, presentations, psycho-educational and support groups, and provides education to patients/family/interdisciplinary team as needed regarding Social Work practice.
Responsibilities
1. Provides direct social work services to patients and families in the assigned clinical area within Acute Care by exploring the emotional response to identify problems, personal concerns, and treatment objectives and facilitating change in coping style, behavior, attitude, and feelings as it relates to the illness and other systemic issues. Participates with patients, families, and community supports and interdisciplinary treatment teams to coordinate services, manage cases, and facilitate meetings.
2. Conducts psychosocial assessments of patients and families by interviewing the patient and/or family, obtaining relevant information, gathering social data, and interpreting the data using a variety of modalities and in accordance with professional standards and clinical policies and practices.
3. Provides consultation to patients, families, and healthcare professionals on issues related to high‑risk factors, recovery, and current community practice relevant to the assigned clinical area in the fields of adaptation to illness and disability, trauma and bereavement, critical incident stress management, protection of vulnerable adults, chemical dependency, and community living and support services.
4. Maintains patient records by documenting assessments, charting patient information, and preparing progress notes and reports in accordance with established standards, policies, and procedures to meet regulatory requirements.
5. Provides leadership in the discharge planning process by assessing care needs and collaborating with interdisciplinary team members to develop and coordinate care plans and community services for patients requiring facility placement and/or home support.
6. Communicates with community health and social agencies, including government ministries, continuing care personnel, and family physicians to assess and manage individuals at risk, make referrals to appropriate resources, and exchange relevant information.
7. Acts as a resource to patients and families by providing information regarding community resources, facilitating referrals, and supporting optimal planning.
8. Participates as a member of local committees and planning bodies by engaging in program and practice evaluation, identifying opportunities for change, and analyzing outcomes to improve service delivery.
9. Maintains records such as workload statistics and prepares reports and recommendations for the Program Manager and Clinical Practice Leader.
10. Attends and leads meetings, in‑services, psycho‑educational and support groups.
11. Participates in research activities and program planning to identify current trends and gaps, administer literature searches, and collect research data for analysis.
12. Engages in professional development by reviewing literature, consulting with experts, and evaluating practice to improve care.
13. Performs other related duties as assigned.
Qualifications
Education and Experience
Bachelor’s Degree of Social Work from an accredited school of Social Work, plus one year of recent related experience in a hospital setting and/or public agency. Current full registration with the BC College of Social Workers.
Skills and Abilities
- Comprehensive knowledge of emotional and mental health issues related to illness and injury, and patient/family cultural dynamics.
- Basic counseling skills including crisis counseling, brief therapeutic interventions, and loss and grief applicable to families, young adults and seniors.
- Ability to provide consultation to patients, families, and healthcare professionals.
- Ability to develop, coordinate, and deliver in‑services, presentations, and support groups.
- Leadership in interdisciplinary care planning for effective resource utilization.
- Effective referral skills and collaboration with community health and social agencies.
- Knowledge of community, social, health, and welfare programs and related policies.
- Effective communication with patients, families, staff, and community agencies.
- Organization and prioritization of workload in complex environments.
- Physical, emotional, and flexible ability to carry out the duties of the position.
