Clinical Manager - The Path Home Transitional Care Program
The Complex Care and Recovery Program at CAMH is recruiting a full-time, temporary (18 months) Clinical Manager to oversee the Path Home Transitional Care Program.
This position reports to the Clinical Director of ANS, Geriatric and Medical Affairs. Priorities in this position include leading significant strategic initiatives to enhance integration and collaboration of the services within the portfolio, across CAMH improving effectiveness and access to care and throughout the health system. The Clinical Manager will be responsible for defining performance management objectives and establishing operational performance standards while monitoring daily clinical operations of the services.
The Path Home Transitional Care Program offers transitional care support in a post-acute inpatient setting for patients who are designated Alternate Level of Care (ALC) at CAMH. The goal is to support the individual to live in the community either in supportive housing or LTC after treatment. The program operates in partnership with LOFT Community Housing as well as practice areas and inpatient programs across CAMH.
The Clinical Manager will support the mandate and smooth operations of the Path Home Program at CAMH, manage stakeholders' relationships and partnerships, participate in report development and support the ongoing professional development of staff. The Clinical Manager will also strive to create a safe and healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. This position is currently located at the Queen Street site, but may require work arrangements from other CAMH locations.
The successful candidate will have a Master's Degree, preferably in nursing (e.g., Health/Business Administration, Public Health, Research, or a Clinical field), combined with at least 2 years of experience in clinical leadership. Registration in good standing, as a Regulated Healthcare Professional is required as is specialized knowledge of mental health and addictions and specific knowledge of the patient population. Demonstrated ability to effectively supervise the work activities of others within an interprofessional team. You will utilize excellent communication and interpersonal skills to build collaborative clinical teams and to provide leadership. You exhibit strong written and verbal communication skills, are self-motivated, and work successfully both independently as well as in team settings. You will have excellent decision-making and problem-solving skills to intervene in complex situations. Computer literacy including proficiency with Microsoft office suite is required. The ability to work with persons of diverse ethnoracial and cultural backgrounds is also required along with conflict resolution skills. The ability to manage budgetary and statistical data and knowledge of human resource management practices in a unionized environment is also required. Bilingualism (French/English) and/or proficiency in a second language would be an asset.
Please Note: This full-time, temporary (18 months) position is NOT part of any bargaining unit.
