Are you a compassionate and experienced mental health professional ready to lead and inspire a team? Active Pathways is excited to offer an opportunity for a Mental Health Team Leader to join our brand-new 10-bed rehabilitation service, Heathy House, in Halifax. This residential service provides a step-down environment for individuals on their journey to independent living.

This full-time role follows a four-week rolling rota with varied shifts between 6am and 10pm, Monday to Friday, and includes one Saturday shift per month. While the rota provides structure, some flexibility is required to meet service needs, including occasional shift cover. Adjustments can be considered for the right candidate, but flexibility remains essential. You can find the full rota outlined below

Shift Pattern

- Week 1

- Mon–Wed: 9am–5pm

- Fri: 6am–2pm

- Sat: 9am–5pm

- Weeks 2 & 3 Mon–Tues: 9am–5pm

- Wed: 2pm–10pm

- Thurs–Fri: 9am–5pm

- Week 4

- Mon–Tues: 9am–5pm

- Wed: 7am–3pm

- Thurs: 12pm‑8pm

- Fri: 9am–5pm

Heathy House offers supportive residential care in for men with mental health needs. Designed to bridge the gap between hospital care and independent living, Heathy House supports up to 10 residents for 18 to 24 months. Our structured, person-centred approach focuses on building independent living and life skills, enhancing access to education, employment, and community opportunities, and supporting emotional and physical wellbeing. We work in partnership with residents to plan their next steps toward greater independence, promoting resilience, recovery, and personal growth.

About the Role

As Team Leader, you’ll play a key role in delivering safe, high-quality, and recovery-focused care. Working in close partnership with the Registered Manager, you’ll provide day-to-day leadership, mentor staff, promote a positive and inclusive culture, and ensure our residents are supported to reach their full potential.

Key Responsibilities

- Lead and support the team each day, including training, mentoring, and supervising senior staff.

- Ensure safe, high-quality, person-centred care that meets CQC standards and legal requirements.

- Promote recovery-focused support and keep care plans and documentation up to date.

- Help manage rotas, staffing, and budgets, and support with petty cash and service user finances.

- Oversee health and safety, infection control, incident reporting, and the general upkeep of the service.

- Support recruitment, inductions, staff training, appraisals, and day-to-day HR processes.

- Help assess referrals, attend reviews, and maintain good communication with families and professionals.

- Encourage learning and development, promote equality, and support a positive team culture.

- Step in for management when needed and take part in the local on-call rota.

Requirements

- Proven experience supporting individuals with complex mental health needs

- Strong leadership, mentoring, and organisational skills

- Knowledge of CQC standards, safeguarding, and clinical governance

- A recovery-focused mindset and commitment to person-centred care

- Experience with audits, care planning, and leading a team effectively
