Position Overview

The Clinical Educator – Integrated Programs drives best-in-class clinical competencies across a high-performing interdisciplinary team. Using an interprofessional practice mindset, this role plays a pivotal part in shaping education, training, and mentorship strategies to ensure team members meet the evolving needs of diverse populations.

KEY RESPONSIBILITIES/ACCOUNTABILITIES:

Education & Training (35%)

- Develop and deliver tailored training, education, and orientation programs for interdisciplinary team members.
- Create and maintain dynamic training resources, such as e-learning modules, clinical guides, and hands-on workshops.
- Facilitate onboarding for new hires, ensuring consistent understanding of integrated care principles and clinical processes.

Competency Assessment & Development (25%)

- Support operations leaders to assess team members' clinical competencies and identify areas for professional development and upskilling.
- Establish a framework for ongoing competency evaluation and support to maintain high standards of care delivery.
- Design and implement education programs and resources that address competency gaps and promote continuous professional growth.

Clinical Mentorship & Guidance (20%)

- Provide mentorship to team members, offering guidance on clinical decision-making and best practices.
- Conduct on-site visits to observe, coach, and support the delivery of care in home and community settings.
- Act as a clinical resource, troubleshooting challenges and fostering a culture of learning and collaboration.

Clinical Process Development (10%)

- Collaborate with leadership and the interdisciplinary team to develop evidence-based clinical protocols and workflows.
- Ensure clinical processes prioritize patient safety, equity, and efficiency while aligning with the program's goals.

Stakeholder Collaboration (10%)

- Partner with external stakeholders to align training and processes with provincial standards and best practices as well as funder requirements.
- Engage with patients and families to understand their needs and inform program improvements.

What you will need to be successful:

- Registered Nurse (RN) with current registration in good standing (preferably in Ontario).
- Bachelor's and/or Master's degree in Nursing, Education, or a related field is an asset.
- 5+ years of clinical experience, with a background in acute care, home care, or community care preferred.
- Experience in clinical education, competency development, and/or leadership roles.
