COMMUNITY HOSPICE PALLIATIVE CARE NURSE (RN)

Registered Nurse (RN) – Community Hospice Palliative Care Team (designated bilingual position)

Position Type: Part-time (averaging 30 hours / 4 shifts bi-weekly)

Schedule: Alternate Monday & Thursday (8:30 am – 4:30 pm) and every second weekend (Saturday & Sunday, 8:30 am – 4:30 pm)

Contract Expiry: March 2026

Must be available to work holidays.

1st week: Sunday & Thursday; 2nd week: Monday & Saturday.

POSITION SUMMARY

As part of the Hospice Palliative Care Team (HPCT), the Registered Nurse is responsible for providing telephone support / consultation to clients that have a life limiting illness and are in the last year of life, and their caregivers.

MAJOR AREAS OF RESPONSIBILITY

- Provides comprehensive telephone assessments with a holistic approach, to address pain and symptoms experienced during multiple illness trajectories of life limiting illness

- Performs assessments, care plan development and ongoing monitoring of each client in accordance with the established Maison McCulloch Hospice (MMH) Policies and Procedures

- Reports all findings to the assigned Most Responsible Provider (MRP) whether this is the Physician or Nurse Practitioner

- Schedules home visits for these providers so that client’s ongoing needs are met

- Collaborates with the North East Cancer Centre, “Ontario Health atHome” and their contracted nursing providers

POSITION REQUIREMENTS

- BScN in Nursing with a certificate of competence with the College of Nurses of Ontario

- Minimum of 5 years full-time equivalent nursing experience

- Ability to communicate effectively with clients and family in person or via telephone and troubleshoot any of their concerns

- Receptive to ongoing education in Hospice Palliative Care

- Must have valid driver’s license and access to a vehicle

PREFERRED

- Recent minimum of 2 years full-time equivalent in Hospice Palliative Care (HPC) experience

- Certified or receptive toward achieving certification in Hospice Palliative Care with the Canadian Nurses Association (CHPCN(C) designation)

- Proficiency in French, (verbal and written)

- Experience providing telephone nursing in a sensitive and compassionate mannerAbility to develop a plan of care with clients, primary caregivers and community providers

- Strong knowledge of community resources and ability to refer as needed for various community services

PERSONAL SKILLS

- Demonstrates at all times clinical autonomy

- Working knowledge of computer software applications Microsoft Office, (Excel, Word, Outlook)

- Self-directed with exceptional organizational and interpersonal skills

- Professionalism in all work settings (community in home setting and Maison McCulloch Hospice)

- Ability to maintain confidentiality when sharing and discussing personal client information with the inter‑professional team

SALARY & APPLICATION
