- YOUTH WELLNESS HUBS ONTARIO (YWHO) NURSE PRACTITIONER

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking one Nurse Practitioner (NP) for the Thorncliffe Park YWHO, to join our team in a permanent part-time role. TNO is a great place to work and we make a difference in the communities we serve.

The Nurse Practitioner (NP) will work as a member of a diverse interdisciplinary team to support youth health and wellness, including those facing mental health and substance use challenges. The NP provides leadership and promotes excellence in care from a primary care perspective in all areas of Advanced Practice Nursing.

- Provides comprehensive episodic primary care to individuals’ ages 12-25 years of age.
- Performs assessment that includes completing a health history, collecting all relevant information pertaining to the presenting problem.
- The NP will collaborate with community partners, family, caregivers with the consent of the patient to collect collateral information that can help to inform the assessment, diagnosis and treatment plan.
- Provides care for youth and/or monitors the ongoing therapy of youth. Where necessary and appropriate, consults with other members of the interdisciplinary team to provide interventions and comprehensive care, including prescribing drugs, ordering laboratory tests and emphasizes health promotion, education and disease prevention with youth.
- Make referrals where necessary to other community providers.
- Employs nursing communication skills in all interactions (e.g., active listening, respectful and culturally appropriate verbal and non‑verbal communication, compassion, empathy, etc.).
- In collaboration with youth, support the use of YWHO standardized measures to support measurement‑based care to provide health and services, monitor progress, and improve youth outcomes. Participates and/or leads in complex care planning, including assessing, evaluating, amending plans of care after ongoing evaluation of the youths’ needs in collaboration with the interprofessional team.
- Performs selected diagnostic and therapeutic procedures and pharmacological agents and therapeutic measures within scope of practice.
- Identifies and communicates a medical diagnosis to the youth.
- Integrates psychotherapeutic approaches into their clinical visits.
- Provides leadership advocating for the practice changes to enhance safe patient care.
- Provide services and support to youth and their families in a way that is consistent with the values of the YWHO Model.
- Provide access to services both in person, remotely (off site) or virtually (online, phone) as needed.
- Works collaboratively with Hub leadership to identifying scholarly activities.
- Develops and implements; or participates in and promotes scholarly activities.
- Collaborate with Youth Wellness Hubs, YWHO Provincial Office and other clinical partners with developing, participating in knowledge exchange activities.
- Seizes opportunities for continued professional development and engages in informal and formal learning opportunities.
- Seeks opportunities to engage in mentorship activities.
- Participates when and where possible in the academic community. Becomes cross‑appointed to the Faculty of Nursing associated with the community the hub is based in, when and where possible.
- Provides health education in areas such as harm reduction, gender affirming care, sexual health collaboratively with others from the Youth Hub team, partners and the larger Network.
- Collaborates with YWHO Hub leadership team to implement the Nurse Practitioner role fully based on legislative/regulatory requirements; professional standards and guidelines; organizational policies and procedures; and individual knowledge, skills and judgment.
- Acts as a mentor and role model for clinical practice initiatives.
- Accountable to align activities and performance with strategic goals and objectives of YWHO Hubs.
- Advocates to influence primary health care practice, policy and outcomes.

Vaccination Requirement – You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type Part‑time permanent position (17.5 hours/week)

The ideal candidate will hold a Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) with a focus on Primary Care and be registered with the College of Nurses of Ontario (RN‑Extended Class) in good standing. They must be authorized to prescribe controlled substances and have completed the Nurse Practitioner exam. Clinical expertise in youth health, including experience with children, youth, and young adults, is required, as well as proficiency in mental health assessment and treatment. Experience in community healthcare, particularly with youth, is essential, along with strong critical thinking, problem‑solving, and leadership skills. A commitment to continuous learning, including subspecialty education in Mental Health and Addictions, Gender Affining Care, and quality improvement, is required. Strong communication, public speaking, and advocacy skills are also necessary.
