Clinical Systems Educator

Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient's life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. JOB SUMMARY
Reporting to the Director, Data and Digital Health Innovation, the Clinical Systems Educator is considered the Subject Matter Expert, technology focused, and is responsible for providing training on applications related to the provision of clinical care to the staff within Integrated Care Solutions. The clinical systems educator will also support project coordination of digital products.
Duties and Responsibilities:
1. Train new and existing staff on the clinical documentation system, interRAI assessment tools, and other Bayshore applications (including workflow management tools, patient engagement applications, Yardi, Procura, etc.)
2. Prepare support materials (e.g. user guides, educator guides) for existing and new applications
3. Partner with clinical operations team and digital health team to update training materials and methodologies to reflect current field practices and system statuses
4. Modify and configure systems as needed
5. Develop and conduct training sessions as required
6. Create/update/communicate training schedules to ICS staff
7. Analyze feedback from ICS staff and provide input to assist in the improvement of training methodologies and initiatives for existing and new clinical program implementations
8. Troubleshoot device/application issues and provide guidance to users
9. Participate in ongoing internal and/or external continuing education activities
10. Learn new applications to update training materials and provide training on newly acquired applications
11. Participate in project implementations from a training perspective as required
12. Provide project management support:
13. Apply standard project management best practices and methodologies to effectively manage program and project objectives.
14. Support the PM with planning of project meetings including creating agendas and presentations, scheduling meetings, and taking meeting minutes.
15. Maintain project documentation, including project charters, project plans, communication plans, meeting minutes, evaluation plans, sustainability plans, and progress reports.
16. Follow-up with stakeholders to ensure completion of deliverables are tracking towards deadlines.
17. Collaborate with cross-functional teams to ensure effective communication and alignment
18. Use project management software as needed
19. Support the leadership team with change management and develop plans for sustainability
20. Adhere to Bayshore Policies and Procedures
21. Participate in quality activities and continuous improvement initiatives in keeping with Bayshore's Quality Management System
22. Participate in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns
23. Maintain confidentiality of client and corporate information and discuss same only with appropriate Bayshore personnel
24. Assist in data gathering and analysis activities
25. Help facilitate data and workflow auditing
26. Complete other tasks as requested
27. Travel to various sites may be required
Education and Training
1. Diploma in Registered Practical Nursing or relevant course of study; completion of relevant courses/certificates
2. Possess current valid license as a registered nurse or registered practical nurse
3. Adult learner/education certificate/training or willing to obtain in 2 years
4. Health informatics certificate or willing to obtain in 2 years
5. Valid G driver's license
Experience
1. Experience as a regulated health professional in home care/acute care settings. Relevant field experience preferred
2. Previous experience assessing patients as well as training and mentoring staff of various ages
3. Experience leading or participating in change initiatives and familiarity with project management principles.
4. Excellent knowledge of Microsoft Office products
5. Knowledge of electronic documentation systems and other applications is an asset, such as, CRM, eLearning.
6. Well versed in PC, Android/Apple OS and comfortable in using consumer devices, IoT, web applications, cloud computing, web conferencing software
