**SCOPE**:
The **Mindimooyenh Health Outreach Worker **will assist community members with accessing health services in a timely, safe, and culturally appropriate manner. They will support community members with bookings, attending appointments, filling out consent forms (documentation), support administrative functions related to the health clinic and assist with follow up on health care access and health related issues, across Ontario. The **Mindimooyenh Health Outreach Worker **is responsible for assisting in developing, implementing, documenting, and advertising land-based and promotional health programming, including community events as well as within the mobile unit team. They will foster and promote cross-cultural awareness and sensitivity for health practitioners and provide a warm, welcoming, and supportive experience to everyone. They will assist with driving, care and health clinic related maintenance of the mobile unit when needed and will perform general office management duties providing organized proactive administrative, technical, and logístical support. perform general office management duties providing organized proactive administrative, technical, and logístical support.

**QUALIFICATIONS**:

- A minimum diploma in the Health, Human Services field from an accredited College, and 1-3 years’ work experience in the relevant area.
- Knowledge and experience in community-based program services delivery. Previous experience of successful working experience in providing health support, cultural programming and/or customer service is preferred and considered an asset.
- Excellent phone etiquette skills combined with excellent relationship building skills and the ability to maintain positive relationships with all internal and external stakeholders.
- Knowledge of and the ability to develop, implement and evaluate programming.
- Proactive and able to make decisions that work to resolve and support all inquiries.
- Knowledge of Indigenous support services available within the community(s).
- Knowledge of and connection to service agencies in the location of service delivery.
- Ability to network with other community agencies/services/organizations.
- Demonstrated experience providing services to Indigenous families and communities.
- Must be dependable and punctual with strong attendance history, able to follow instructions, respond to management direction and be able to improve performance through management feedback.
- Capacity to solve problems and to see different perspectives.
- Ability to plan, organize and manage time effectively.
- Highly motivated individual with the ability to work with mínimal supervision.
- Excellent communication skills and an ability to follow through on assigned duties independently with strict deadlines.
- Knowledge of Not-For Profit Organizations.
- Proficient working knowledge of MS Office Software, internet, and general office equipment.
- Ability to speak an Indigenous Language is considered an asset.
- Maintains complete confidentiality.
- Comfortable working in a fast-paced working environment and can multitask and prioritize without guidance.
- Flexible with conflicting demands and required needs.
- Thorough understanding of Public Health (Community Wellbeing) Systems and Indigenous health care systems.
- Valid G Class Driver’s License and access to a reliable vehicle with appropriate insurance.
- Current, valid vulnerable sector criminal records check. Valid 1st Aid/CPR.
- This position requires travel and consists of evening and weekend work.
