**JOB SUMMARY**:
Carpenter Hospice is an 11-bed home in the heart of Burlington that offers resident-based care to individuals in the last stages of their lives, and community-based programs for individuals dealing with grief or a life-limiting illness. It is an exciting time to be joining Carpenter Hospice as we expand our resident and community services in Burlington and surrounding areas.

The Manager, Resident Care is a member of the leadership team and is responsible for ensuring professional, evidence based, client-centered care for clients admitted to the residence for end-of-life care. The incumbent will oversee care and operations provided by staff and volunteers, fostering a team culture. This position also provides direct client care as required. All responsibilities and duties are fulfilled in accordance with the vision, mission, and strategic direction of the organization, and in accordance with relevant legislation.

**KEY RESPONSIBILITIES**:

- Ensures services are guided by a model of care that is holistic, collaborative and aligns with current evidence-based practice
- Promotes continuum of care model through community and palliative care services
- Supports recruitment, selection and hiring of staff as required
- Provides supervision and support to the resident care team
- Coaches, mentors, and supports orientation and/ or facilitates such processes
- Fosters an environment where staff are encouraged to keep up to date with best practice learning and personal/professional development activities through supervision, performance development and reflective practice
- Implements resident services and delivery models which are consistent with client and clinical evidence-based practice
- Fosters inclusion of clients and care partners to be involved in the process of care and improvement opportunities
- Develops and implements tools, guidelines, and reporting mechanisms to monitor quality and risk
- Integrates sound risk management practices into clinical services and decision-making processes
- Supports client safety culture by ensuring safety for all clients is embedded in the client service delivery model
- Ensures policies, procedures, protocols, and training are in place for such issues as Infection Prevention and Control, medication, and equipment management
- Develops, implements, reviews, evaluates, and updates clinical processes, procedures and supporting documents
- Collaborates with the Leadership Team to develop and provide oversight to quality assurance/improvement and risk management policies and programs
- Participates in the development of program and corporate policies, standards, operating plans, budgets, and funding proposals
- Attends, supports, and is involved with work activities of inter-hospice and organization committees to create opportunities for improved service integration and collaboration

**THE IDEAL CANDIDATE**:

- Nurse Practitioner (RN(EC)) or Registered Nurse (RN)
- Registered Practicing RN with the College of Nurses of Ontario
- Minimum five (5) years of healthcare experience in diverse and progressively responsible positions
- Minimum three (3) years of experience in a leadership role, including experience in palliative care
- Master’s degree in health-related field an asset
- Demonstrates leadership ability and style that inspires excellence, innovation and teamwork
- Demonstrates understanding and experience in health system planning, health care service delivery, organizational leadership, fiscal management, and program development
- Commitment to professional development and continuing education
- Significant knowledge of current and emerging issues pertaining to palliative care at a local, regional, provincial, and national level
- Experience in resident-care support in Palliative Care is preferred
- Effective communication skills including listening, presenting, articulating conveyance of messages in an honest, open, transparent, and straight-forward style
- Exceptional relationship and consensus building abilities and facilitation skills
- Excellent team player with high energy and enthusiasm
- Demonstrated exceptional personal integrity, ethics, trust, humility
- Demonstrated leadership skills including inspiring and motivating staff to achieve outcomes, demonstrating commitment to the mission and values

**OTHER REQUIREMENTS**:

- Ability to work flexible hours including evening hours and weekends to meet the needs of the hospice.
- Ability to travel for local, regional, and provincial meetings
- Required to provide On-Call Manager Support evenings/weekends on rotation with other members of the Leadership Team, approx. every 5 weeks
- Required to provide a satisfactory current criminal reference check (CPIC) including for Vulnerable Populations prior to hire
