This unit is dedicated to providing collaborative care for adult and geriatric patients who require therapy, reactivation, and transition planning to successfully transfer to their community or other supportive environments.

The Reactivation Care Unit focuses on patients who no longer need acute care but still require restorative care to aid their transition home or to other settings. As a member of our dedicated interprofessional team, you will deliver patient-centered care based on best practice principles. Our team will work closely with patients, families, and caregivers to provide reactivation therapy aimed at enhancing and optimizing patients' functional abilities and engagement in daily routines.

**Job Profile**:
The Rehabilitation Assistant (RA) will provide high quality consultation, assessment and intervention for a variety of medicine patients within an Interprofessional Collaborative Practice philosophy. Being part of this team means being part of an exciting new initiative to help us support patients along their continuum of care and to shape how care is delivered.

The RA will be responsible for the following, but not limited to:
- Assist patients with daily activities under clinical expert guidance.-
- Aid in walking and exercise routines for both individuals and groups.- Provide and modify equipment and materials for patient treatment.- Train patients in the use of equipment and develop strong patient relationships.- Assist with patient transportation, lifting, and transfers.- Prepare materials, maintain records, and ensure treatment areas are clean.- Participate in conferences and meetings as required.

**Qualifications**:
- Post-secondary diploma from a recognized Rehabilitation Assistant program (e.g. combination OTA/PTA Program) required.- Current Basic Life Support (BLS) certification required- Minimum 1 year experience working as a Rehabilitation Assistant in a rehab or acute care setting required.- Knowledge/experience with neurologically impaired patients, medicine patients, wheelchairs & mobility aids preferred.- Demonstrated competency in proper patient handling, transfers and safe mobilization- Basic computer skills preferred.- Must have excellent working knowledge of the English Language both verbal and written.- Must have strong communication and interpersonal skills.- Must be able to prioritize and organize duties, and work well independently.- Must be able to provide patient care in both group and individual settings.- Must be able to provide support to multi-disciplinary team.- Must be able to meet the physical requirements of bending, transferring, assisting and supporting patients.- Satisfactory attendance record.
