**Job Description**:
The **Allied Health Services (AHS) **team, under Professional Practice consists of Regulated Healthcare Professionals including: Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, Registered Dietitians, Respiratory Therapy, as well as unregulated Rehabilitation Assistants. Each discipline is focused on driving evidence informed practice to promote high quality patient and family centered care. The team has a professional practice leader (PPL) model for each regulated allied health discipline which promotes and enhances the interprofessional team. Each discipline has their own PPL with dedicated time to enhance their communities of practice, represent their respective disciplines at committees, create policies & procedures and lead corporate level initiatives.
We are currently seeking a **full time permanent Social Worker** to support our acute care medical and surgical units. This position will focus on supporting our Cancer and Palliative Care Unit.
**Education**:
- Graduate of an accredited School of Social Work with a Masters of Social Work
- _Required_
- Current registration with the Ontario College of Certified Social Workers/Social Services Workers -_Required_
- Sound knowledge of related legislation as applicable to serviced population
- _Required_
**Experience**:
- Recent Social Work experience involving an acute care medicine/surgical settings - _Required_.
- Recent Social Work experience involving cancer and palliative patients - _Preferred_.
- A strong theoretical and practical knowledge of social worker role and scope in promoting independence and assistance in acute care/ setting
- _Preferred_.
- Strong knowledge of community services, supports and experience related to facilitating family meetings and discharge planning
- _Required_.
- Strong knowledge of legislation applicable to hospital social work practice (Substitute Decisions Act, Child and Family Services Act, Public Hospitals Act).
- Experience with quality improvement initiatives or projects an asset and experience with supporting patient and families through Medical Assistance in Dying
**Competencies**:
- Ability to function co-operatively as part of an interprofessional assessment team
- Possess comprehensive assessment, treatment, and documentation skills related to biopsychosocial components of well-being
- Possess excellent interpersonal and communication skills
- Demonstrate sound clinical judgment, flexibility and initiative
- Ability to communicate effectively with patients, families, caregivers, and other professionals
- Superior organizational and time management skills are essential to management of the caseload
**Other**:
- Ability to learn about, from, and with other members of the patient care team to foster a strong interprofessional model of care.
- Demonstrate an ability to appreciate the importance of, and be open to new concepts and the ability to trial new approaches.
- Ability to provide direction and synthesize information from other disciplines to create a comprehensive care plan and present recommendations to the most responsible physician.
