**About the Opportunity**:
Reporting to the Director of Continuing Care, the Access and Flow Manager is responsible for providing zone and operational leadership for continuing care access and flow, with a client centred, collaborative practice model for client care. Will have responsibility to lead and engage stakeholders in the development, implementation and evaluation of improvement strategies to ensure efficient and effective utilization of resources within the zone across hospital and community.
Working collaboratively and consultatively with patients/clients, families, management, acute care staff, physicians, physicians’ offices, community partners, facilities and service providers, the Zone Access and Flow Manager applies critical thinking to address challenges to achieve the ultimate goal of high quality, safe and cost-effective services to better support the patient experience and flow within the zone.
**About You**:
We would love to hear from you if you have the following:
- Baccalaureate degree in a healthcare field and a minimum of 5 years recent and related leadership/management experience is required
- Must be currently registered with an applicable health regulatory body
- Demonstrated leadership skills in a healthcare environment and ongoing professional development
- Progressive experience in management and administration in a health care/ community setting, including financial and human resource management.
- Demonstrated commitment to a Home First philosophy that fosters independence for those who require care and to the utilization of evidence to inform practice
- Experience with continuing care an asset.
- Intermediate level computer skills (SEAscape; MSOffice; PathWays; Tableau; Patient Flow, Dashboard; SharePoint **and where exist** Patient Audit System; Business Objects Patient Flow Reporting.
- Experience with bed management/Care Coordination C3 software an asset
- Demonstrated knowledge and experience managing in a community case management environment
- Demonstrated ability to build and maintain effective collaborative relationships and partnerships with stakeholders and clients
- Strong human resource management, financial management, interpersonal, and communication skills
- Competencies in other languages an asset, French preferred

**Hours**:
- Permanent Full-time Position; 75 hours bi-weekly

**Compensation and Incentives**:
$41.48 - $55.11 Hourly
