Job Summary
Coordinate, organize and facilitate the ‘Social Prescribing for Mental Health’ project at the Centre. The objective of this project is to enhance community-based mental health supports for clients to decrease isolation and increase positive health outcomes by piloting Social Prescribing tools.

Job Specific Responsibilities
- Build organizational knowledge on Social Prescribing
- Lead the creation of process maps for referrals, promotion/communication (internal and external) and client journey
- Engage internal and external stakeholders/partners
- Engage in relevant Communities of Practice
- Provide regular updates at appropriate team meetings
- Gather feedback from clinical staff and at-risk groups in the PQ & SN satellite catchments including clients who:

- self-identify as struggling with mental wellness
- reach out for mental health-related supports
- are at-risk for adverse mental health
- are disproportionally impacted by the pandemic due to race, language, income, status or digital access
- Audit potential tools for assessment, monitoring and evaluation in collaboration with PQ teams and the Alliance for Healthier Communities.
- Engage in the referral process
- Ensure clients are linked to social connections that enhance mental wellbeing.
- Project - related data collection and management.
- Prepare a final report including tracked data and evaluations on effectiveness of the pilot project.

- Contributing to the Centre’s activities to collect, analyze and report on data and relevant information, and participate in research.
- Supporting the Centre’s volunteers and placement programs.
- Promoting awareness of and participation in Centre activities.
- Contributing to the Centre’s work by participating in meetings and committees, when applicable.
- Working during both regular and extended hours of operation in locations identified by the Centre.
- Contributing to the Centre’s efforts to secure and maximize resources for current and new programs, services and activities.
- Working in a manner that supports the Centre’s Occupational Health and Safety Policies and Procedures.

**Qualifications**:

- Bachelor’s degree in health or social sciences, or equivalent combination of experience and education.
- Experience in community health and client referrals
- Experience in cross-team collaborations
- Excellent organizational skills regarding timelines and e-filing.
- Excellent communications and interpersonal skills.
- Experience working with multicultural communities in one-on-one as well as group settings.
- English is essential, other languages considered an asset.
- Able to work in a hybrid setting: in-person and virtual.
