We will soon be recruiting for a Community Navigator & Community Assessment Caseworker. The Halifax Regional Office delivers a wide array of support and services to criminalized persons in or returning to HRM. Our Transition Team works collaboratively to support needs, interests and goals across the social determinants of health as people navigate incarceration, their release and/or (re)integration in the wider
community. The Transition Team uses an anti-oppressive framework to support the identification, prioritization and planning of appropriate case management for men.

**Position & General Accountability**:
The Community Navigator & Community Assessment Caseworker will:
∙ Utilize the Social Determinants of Health to provide systems navigation and integration support to criminalized and non-criminalized men in the community.
∙ Employ a lens that is trauma-informed and culturally sensitive to help men navigate systems barriers and support connection with appropriate community resources and support networks.
∙ Provide community leadership with cultural humility/competency for working with clients. The incumbent must possess a firm understanding of support services and resources available to clients in HRM.
∙ Will work in close coordination and collaboration with other Transition Team members and will engage in intensive, supportive case management processes.
∙ Report to their immediate supervisor (the Transition Team Lead) for support in daily tasks/guidance.
∙ Will complete interviews, in person and over the phone as well as write reports to submit to Correctional Service Canada.
∙ The Transition Team is accountable to the Halifax Regional Director.
**Position Type**:Full-Time Term
∙ Rate of Pay & Leave Entitlement TBD. Minimum rate $21/hr.
**Location**:
The position may be based out of The John Howard Society of Nova Scotia’s Provincial Office and Halifax Regional Office (HRO) at Suite 1, 541 Sackville Drive, Lower Sackville NS. Please note stairs must be used to access the HROs work site. Your schedule may include a combination of in-office and ‘work from home’ days, depending on organizational needs. In addition, this
position requires daily travel throughout HRM to meet with clients and community partners.
**Working Conditions**:
∙ The position requires moderate amounts of walking, climbing stairs, standing, sitting, lifting,
carrying, and both scheduled and unscheduled interactions with others.
∙ The position is fast-paced and demanding, requiring a reasonable amount of mental effort to analyze information, problem solve and support people.
∙ Personal Protective Equipment (in relation to Covid-19) will be supplied (masks, gloves, disinfectant).
∙ The equipment and resources necessary for carrying out your work will be supplied (ex: computer, stationery supplies, etc.), according to regular standards of work within JHSNS.

**Specific Duties & Responsibilities**:

- Working closely with JHS’ internal programs, as well as a variety of government and community partners.
- The CN connects with clients to ascertain their individual needs, interests and goals in order to assist them in the community.
- The CN will work with clients to develop individual support plans that explore and engage available resources across the social determinants of health, including financial supports, shelter/housing, basic needs (food, personal care items), wellness, community support, cultural support, employment options, among others.
- The CN will take a person-centered approach to their work, which includes using culturally appropriate/sensitive approaches for service delivery.
- The CN will work within the scope of support available through JHSNS and will engage the expertise of other community and government resources as needed, as appropriate.
- The CN will maintain regular connections to a variety of communities within Nova Scotia, including the African Nova Scotian Community, BIPOC Community, LGBTQ2s+ community.
