Ontario Health Teams are a new way of organizing and delivering care that is more connected to patients in their local communities. Children’s Centre Thunder Bay is the administrative support partner to the Noojmawing Sookatagaing Ontario Health Team.

The OHT is currently seeking to fill the following position:
Integrated Care Pathways Project Manager

Full Time Contract Position

The Integrated Care Pathways (ICP) Project Manager plays a pivotal role in managing projects and implementing the work of the Noojmawing Sookatagaing Ontario Health Team (OHT). These integrated pathways aim to provide coordinated, person-centred services for individuals with chronic diseases, ensuring seamless care throughout their entire journey. Working with diverse stakeholders including primary care, community care, and people with lived and living experience will be essential to enhancing patient outcomes through this initiative.

This role requires a seasoned project management professional with strong leadership, communication, and organizational skills to ensure the successful execution of initiatives that improve healthcare delivery and patient outcomes in the District and City of Thunder Bay. A commitment to health equity and an understanding of the multiple facets that encompass integrated care is essential.

**Qualifications and Experience**:

- Bachelor's degree in project management, healthcare administration, business administration, or related field. Master's degree is a plus.
- Project Management Professional (PMP) certification preferred.
- Proven experience in successfully managing complex projects in healthcare or related fields.
- Valid Driver's license and ability to travel within the City, District and region.
- Ability to work flexibly based on project needs.
