The **Program Assistant - Recreation - ICCP (PA-R-ICCP)** has ownership of the planning, promoting, implementing and evaluating of recreational and cognitive activities of the patients within the ICC Program. The PA-R-ICCP is primary responsible for attending dialysis (or other) patients in their homes, and engaging them in various dexterity and cognitive activities to further their abilities and confidence with eventual self-administered care. The PA-R-ICCP monitors and observes the patients for extended periods of time, and reports any abnormalities to clinical staff and/or Program Leadership as necessary. The PA-R-ICCP ensures that recreational activities are in line with the care plan of the patient, and provides Program Leadership with continuous feedback and input about the changing recreational (or other) needs. This role is also responsible for assisting with program reports as necessary, and for working within the ICC Program framework.

**CORE DUTIES AND RESPONSIBILITIES**
- Attend initial, or follow up, patient visits with a Clinical Staff member
- Complete client assessments within the scope of recreational activity needs
- Develops and delivers patient recreation plan, in keeping with specific needs of the patient
- Reviews and follows care plans based on assessment results
- Observes, monitors or reports on any clinical issues to the primary care nurse or Program Leadership
- Monitors, verifies, troubleshoots, and reports on patient equipment during the recreational visits
- Reports and/or responds in keeping with standard protocols to any emergencies which occur during the recreational visit
- Completes any required reports or documentation as directed by the Program Leaders
- Reports any changes to patients physical or cognitive condition to the primary care nurse or Program Leaders
- Supports the patient post clinical care, as directed by the primary care nurse or the Program Leaders
- Ensures that the equipment or area in which the recreational activities take place is cleaned and maintained with good housekeeping and IPAC practices, including proper disposal of any waste as instructed the end of each visit
- Collaborates with the primary care nurse or Program Leaders at least 1-3 times per week, shares information and updates as necessary
- Completes Delegated Acts, only as trained and instructed by the primary care nurse or clinical program leaders
- Positively promotes recreational activities with the patient and their family / caregivers
- Positives engages the patient in their journey towards increases independence and self-care
- Communicates clearly, and received input from, the patient about their recreational activity plan and the patient’s recreational preferences
- Encourages a variety of activities to increase dexterity, cognition and/or confidence
- Assists with the coordination of external activity/ service providers as necessary
- Provides input into tracking and reporting recreational activity costs

**KEY EDUCAITON AND EXPERIENCE**
- College/university level diploma or certificate in Recreational, Occupational or Physiotherapy Assistant or related education, **required**:

- 1-2 years of related working experience, **an asset**:

- Previous experience in a clinical or home care setting, **an asset**:

- Current First Aid Certificate, **preferred**:

- Current CPR Certificate, **preferred**:

- Training in Gentle Persuasion Approach or another non-violent crisis intervention, **an asset**:

- Proficient computer, technology, and equipment skills, **required**:

- Previous experience working with dialysis patients, **an asset**:

- Willingness and ability to travel on a daily basis**, required**
