**CND OHT Lead, Population Health and Special Projects**

(40 hours/week, Permanent)

**What We are Looking For**:
The Lead, Population Health and Special Projects will support the work of the Connecting Care Advisory Co-Design Group and associated work streams. This role will provide project management support, as well as strategic guidance to support the sustainability of the implementation of CND OHT initiatives. This position will involve close collaboration with members of the CND OHT staff support team (including the Manager, Performance Monitoring and Quality and Digital Health Lead), leaders across the CND OHT, primary care clinicians and external partners.

**Qualifications**:

- University degree in in a related field (i.e. public health, health promotion, health sciences, sociology or health/business administration) or an acceptable equivalent combination of education and experience
- Experience in project management, process/quality improvement and/or population health is an asset
- Experience in clinical settings, especially in working with equity-deserving communities, is an asset
- Experience in supporting programmatic design and development, especially in Mental Health and Addictions and Geriatrics is an asset
- Demonstrated written and oral communication skills
- Ability to work under pressure and tight timelines
- Ability to maintain privacy
- Proficiency in Microsoft Office
- Ability to work flexible hours

**Position Duties and Responsibilities**:

- Manage the planning and implementation of various population health management projects in support of the CND OHT priority populations (frail older adults, mental health and addictions). Duties will include preparation and maintenance of project documentation, facilitation/co-facilitation of project team meetings, on-the-ground project operational readiness.
- Lead the development and implementation of a plan to support the Cambridge North Dumfries Ontario Health Team path to designation.
- Lead relationship management amongst project partners and external stakeholders to support the implementation and sustainability of CND OHT population health initiatives.
- Provide meeting coordination and facilitation support to the Chairs of the Connecting Care Advisory and associated Work Streams.
- Participate in Ontario Health webinars and communities of practice on behalf of the CND OHT, as required, to ensure alignment and collaboration in the areas of care transformation, navigation and coordination.

- Starting Salary of $79,595 to $82,404 annually
- 3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment
